Outreach Community Health Worker- Native American Health and Wellness

Department: Outreach
Location: Hood River, OR

One Community Health is seeking to add an Outreach Community Health Worker to our Preventative Health team focused on providing health-related education through outreach to the Native American community. The Outreach-CHW builds strong relationships and trust with our community. By engaging with community members, they will identify barriers in accessing preventative services and will encourage and refer patients to various health and social service programs to address these barriers. Ensures documentation and effective communication to complete reports and summaries as needed. When requested by the supervisor, the Outreach CHW will be trained and certified as an OHP Assister to support clients with health coverage options, including helping to complete insurance eligibility and enrollment forms. Works both independently and collaboratively as part of the PCPCH team. Key services include (but are not limited to) problem-solving; access to insurance; referrals to community resources, education, and leading educational groups. Performs related work as assigned.

Minimum Qualifications:

  • Highschool Diploma or GED.
  • Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families' physicians, and the community.
  • Cultural literacy through experience working with or living as a tribal member.
  • Commitment to providing culturally and linguistically appropriate care coordination with client and family- centered approach.
  • Reacts calmly and effectively in high pressure and emergency situations. Able to use initiative and good judgment to resolve problems and challenges.
  • Requires critical thinking skills and decisive judgment.
  • Familiar with medical terminology and the healthcare system including the Oregon Health Plan and Washington Medicaid.
  • Able to prioritize and organize work according to multiple and, at times, competing timelines.
  • Attends all Community Health worker meetings, conferences and webinars as required to maintain CHW certification or if requested by supervisor.
  • Work alongside patients to increase their ability for self-management and shared decision- making.
  • Can adequately capture all required documentation in web-based data systems within the time frame required per the program.
  • Valid driver's license and clean driving record.
  • Willingness to travel for events and between clinic locations, as needed.

Preferred:

  • Minimum of two years of experience in a health-related or social service-related field.
  • Knowledge of and compliance with HIPAA regulations.
  • Oregon State Community Health Worker certification or Traditional Health Worker Certification.
  • Knowledge of community resources.
  • Epic experience (strongly preferred).
  • Experience working with low-income, multi-ethnic populations.
  • Experience with training and presentations.

Compensation:

  • Hourly/Non-Exempt: Starting at $20.00- $23.00 per hour, dependent on experience.
  • Weekend Differential: This position receives base hourly pay along with an extra $10.00 for each hour worked on weekends (Saturday and Sunday).
  • Mileage Reimbursement: In accordance with OCH Company Policy.
